Serving the Brant/Brantford area as an Access Mechanism, Contact Brant is one of the key access points to Ministry-funded child and youth services. We are the local Community Information Center – the first place to call for community information, and a partner with 211 Ontario. We are the Lead Coordinating Agency, providing Coordinated Service Planning, FASD Coordination and, for the Hamilton-Niagara Region, the OAP Urgent Response Service.
Mission Statement
Vision: Children, youth, and families are connected to and supported by their community.
Mission: Contact Brant makes it simple:
- to find community information and resources.
- for children, youth, and families to get connected to community services.
We support families by bringing services together for a coordinated plan of care.
Our Story
Contact Brant is celebrating our 25th anniversary in 2025!
Contact Brant was established in 2000 as a single point of access mechanism for cross-sectoral child and youth services. It provides one place to call to access services, and supports families not having to repeat their story to multiple services. We are the local Case Resolution Mechanism, coordinating planning and services for children with complex special needs. We facilitate the Residential Placement Advisory Committee (RPAC) according to CFSA requirements. Today, we serve over 2,800 unique children, youth and families annually.
Under this Access mandate, we also provide ‘coordinated information, which led the organization to fulfill the role of a community Information center and, in 2012, provide a local website of community services, www.info-bhn.ca. Last fiscal year, we had 1,612,626 public views of this website. This, in turn, led to a partnership with 211 Ontario to provide the local community records for their provincial services.
In 2016, Contact Brant was named the Coordinating Agency for Brant. We lead Coordinated Service Planning and partner with five other cross-sectoral service providers to provide this service. Last year, 195 received the support. FASD Service Coordination was added to our mandate in 2018, serving over 600 families annually through individual support and capacity-building sessions. In 2021, we were named the Lead Organization for the Hamilton-Niagara Region’s OAP Urgent Response Service and have consistently exceeded targets. Last year, URS received 696 referrals, completing intake and screening for all. We had 470 eligible/active in service, and completed services with 387, which was 8% above our target.
In the fall of 2024, we led the partnership of local agencies to provide previously unmet service needs through the Extensive Needs Service. Last year, the community served 28 children and youth.
Our People/Our Team
We have always valued family-centred services and have a Peer Support Navigator on our team. The URS team also has a Master of Social Work clinician for this interdisciplinary service. Only four years ago, we had a single manager, the CEO, and have grown with the addition of a Clinical Manager of Service Coordination, a Clinical Manager of Autism Services, and our Executive Assistant, who is responsible for business services and the site.
